Item 6 — PURCHASE OF DRUGS AND MEDICAL STORESमद 6 — दवाओं और चिकित्सा भंडार की खरीद
Item 6 buys drugs and medicines and other items of medical stores, that is surgical instruments, dressings, appliances and X-ray films.
  • Whether to procure through the MEDICAL department or the STORES department is decided by the railway, as per the DRUG PROCUREMENT POLICY
  • The delegation is then made accordingly
  • 6.1 GeM ITEMS
    • 6.1.1 by placing a supply order directly by the DIRECT DEMANDING OFFICER on GeM
    • 6.1.2 direct purchase of GeM items when urgently required
    • For both, the delegation may be made in terms of the Board's letter of 28.12.2015, and PART D, STORES MATTERS is referred to
  • 6.2 NON-GeM ITEMS
    • (a) drugs and medicines; (b) items other than medical stores, such as instruments, appliances, dressings, medical consumables and disposables; (c) X-ray films
    • The same Board letter and Part D govern them
  • 6.3 DRUGS OF IRP-1995 OR A LATER EDITION
    • By OPEN TENDER; by LIMITED TENDER in normal circumstances; by limited tender IN EMERGENCY
    • By SINGLE TENDER for a proprietary article, on a certificate about the medicine
    • The certificate says no other firm in the country makes or sells a similar medicine usable in its place
    • That certificate is the PAC, and the entry is numbered 6/c
    • By single tender for items where it has NOT been possible to give that certificate, numbered PAC/6A
    • Procurement of non-disposable tools and plants goes per case under the same Board letter
    • Finance concurrence is necessary, subject to safeguards for the railway's interest
    • Those safeguards are a BANK GUARANTEE or insurance against failure, defective supply and short supply
    So item 6 sends you to Part D, and you should carry the PAC distinction with you.
मद 6 दवाएँ तथा औषधियाँ और चिकित्सा भंडार की अन्य मदें खरीदती है, अर्थात् शल्य उपकरण, पट्टियाँ, साज-सामान और एक्स-रे फिल्में।
  • खरीद चिकित्सा विभाग से हो या भंडार विभाग से, यह रेलवे औषधि खरीद नीति के अनुसार तय करती है
  • प्रत्यायोजन फिर उसी के अनुसार किया जाता है
  • 6.1 जेम की मदें
    • 6.1.1 प्रत्यक्ष मांग अधिकारी द्वारा जेम पर सीधे आपूर्ति आदेश देना
    • 6.1.2 अत्यावश्यकता में जेम की मदों की सीधी खरीद
    • दोनों में प्रत्यायोजन बोर्ड के 28.12.2015 के पत्र के अनुसार हो सकता है, और भाग D, भंडार मामले देखा जाता है
  • 6.2 जेम से बाहर की मदें
    • (a) दवाएँ और औषधियाँ; (b) चिकित्सा भंडार से भिन्न मदें, जैसे उपकरण, साज-सामान, पट्टियाँ, चिकित्सा उपभोज्य और डिस्पोजेबल; (c) एक्स-रे फिल्में
    • इन पर भी वही बोर्ड पत्र और भाग D चलते हैं
  • 6.3 आईआरपी-1995 या बाद के संस्करण की दवाएँ
    • खुली निविदा से; सामान्य परिस्थितियों में सीमित निविदा से; आपात में सीमित निविदा से
    • स्वामित्व वस्तु के लिए एकल निविदा से, दवा के बारे में एक प्रमाणपत्र पर
    • प्रमाणपत्र कहता है कि उसके स्थान पर प्रयोग योग्य वैसी दवा देश में कोई अन्य फर्म न बनाती है न बेचती है
    • वह प्रमाणपत्र पीएसी है, और प्रविष्टि 6/c संख्या से है
    • जिन मदों में वह प्रमाणपत्र देना संभव न हो, उनकी एकल निविदा पीएसी/6A संख्या से है

    अर्थात् मद 6 आपको भाग D भेजती है, और पीएसी का भेद आप साथ लेकर चलिए।

Item 6.4 — ADVANCE PAYMENT FOR OXYGEN AND SPARESमद 6.4 — ऑक्सीजन और कलपुरजों का अग्रिम भुगतान
The sub-item is advance payment for the purchase of consumable essential medical items. Those are items like oxygen or ethylene oxide cylinders, and spare parts for medical equipment NOT appearing in GeM.
  • PCMD: up to RS 20,000 per case per occasion
  • MD (Zonal Hospital): up to RS 5,000
  • DRM, CWM or SAG: RS 5,000
  • SG or JA Grade: NIL; Senior Scale: NIL
  • THE NOTES
    • A request from firms for advance payment along with the order should be DISCOURAGED
    • It is not accepted unless there are exceptional circumstances justifying a departure from the rules
    So an advance is the exception here, and you should say the rules are departed from only on cause.
यह उप-मद उपभोज्य आवश्यक चिकित्सा मदों की खरीद पर अग्रिम भुगतान है। वे मदें हैं ऑक्सीजन या एथिलीन ऑक्साइड के सिलेंडर, और चिकित्सा उपकरणों के वे कलपुरजे जो जेम पर नहीं हैं।

    • यह तब तक स्वीकार नहीं होती जब तक नियमों से हटने को उचित ठहराने वाली असाधारण परिस्थितियाँ न हों
    अर्थात् यहाँ अग्रिम अपवाद है, और नियमों से हटना कारण पर ही होता है — यह आप कहिए।

Item 7(a) to (c) — LOCAL PURCHASE OF DRUGSमद 7(a) से (c) — दवाओं की स्थानीय खरीद
Part (a) is the day to day local cash purchase of a small quantity of medicines, patient-wise, from a panel of suppliers.
  • PCMD: up to Rs 50,000 in each case; MD (Zonal Hospital): up to Rs 30,000 in each case
  • CMS or CMO: up to Rs 20,000 in each case
  • SG or JA Grade: up to Rs 10,000 in each case; a Health Unit In-charge: up to Rs 2,000 in each case
  • PART (b) — BULK PURCHASE OF DRUGS OUT OF STOCK
    • It excludes the low money value items of the annual indent that the PCMD has approved
    • It goes through QUOTATIONS from firms registered at the PCMD's office
    • PCMD: up to Rs 50,000 in each case; MD (Zonal Hospital): up to Rs 30,000 in each case
    • CMS or CMO: up to Rs 20,000 in each case; SG or JA Grade: NIL; Senior Scale: NIL
  • PART (c) — SERUM AND VACCINES
    • PCMD, HOD, SG or JAG: FULL POWERS; MD (Zonal Hospital): FULL POWERS
    • The DRM's column: FULL POWERS; SG or JA Grade: FULL POWERS; Senior Scale: FULL POWERS
    • Local purchase of serum and vaccines is RESTRICTED to what is needed for immediate treatment
    • That means ATS, ADS, ARV or immunoglobulin for a patient
    • Vaccines for a PROPHYLACTIC measure are beyond the scope of this delegation
    So serum and vaccines carry full powers but a narrow purpose, and you should say immediate treatment.
भाग (a) आपूर्तिकर्ताओं के पैनल से, रोगी के अनुसार, थोड़ी मात्रा में दवाओं की दैनिक स्थानीय नकद खरीद है।
  • पीसीएमडी: हर मामले में 50,000 रुपये तक; एमडी (जोनल अस्पताल): हर मामले में 30,000 रुपये तक
  • सीएमएस या सीएमओ: हर मामले में 20,000 रुपये तक
  • एसजी या जेए ग्रेड: हर मामले में 10,000 रुपये तक; स्वास्थ्य इकाई प्रभारी: हर मामले में 2,000 रुपये तक
  • भाग (b) — भंडार में न रहीं दवाओं की थोक खरीद
    • इसमें वार्षिक मांगपत्र की वे कम मूल्य की मदें नहीं आतीं जिन्हें पीसीएमडी ने अनुमोदित किया है
    • यह पीसीएमडी के कार्यालय में पंजीकृत फर्मों से कोटेशन लेकर होती है
    • पीसीएमडी: हर मामले में 50,000 रुपये तक; एमडी (जोनल अस्पताल): हर मामले में 30,000 रुपये तक
    • सीएमएस या सीएमओ: हर मामले में 20,000 रुपये तक; एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य
  • भाग (c) — सीरम और टीके
    • पीसीएमडी, एचओडी, एसजी या जेएजी: पूर्ण शक्तियाँ; एमडी (जोनल अस्पताल): पूर्ण शक्तियाँ
    • मंडल रेल प्रबंधक का स्तंभ: पूर्ण शक्तियाँ; एसजी या जेए ग्रेड: पूर्ण शक्तियाँ; वरिष्ठ स्केल: पूर्ण शक्तियाँ
    • सीरम और टीकों की स्थानीय खरीद तत्काल उपचार की आवश्यकता तक सीमित है
    • अर्थात् रोगी के लिए एटीएस, एडीएस, एआरवी या इम्यूनोग्लोबुलिन
    • रोग से बचाव के उपाय के टीके इस प्रत्यायोजन के दायरे से बाहर हैं
    अर्थात् सीरम और टीकों में शक्ति पूर्ण है पर प्रयोजन सँकरा, और आप 'तत्काल उपचार' कहिए।

Item 7 — WHEN A LOCAL PURCHASE IS ALLOWEDमद 7 — स्थानीय खरीद कब चलती है
The notes under item 7 say when local purchase may be resorted to at all.
  • Non-supply or DELAYED SUPPLY against the annual indents
  • The annual supply is consumed EARLIER than scheduled
  • A specific brand is needed for an individual patient out of brand loyalty, even where a different brand is available
  • That needs the treating doctor's approval, and the practice is to be avoided altogether, or else minimised
  • Drugs not in common use, with SHORT SHELF LIVES, which are therefore not kept in stock
  • Items left out of the indent because they are newly introduced items of technology
  • Or because a NEW DISEASE PROFILE has come to the surface that was not anticipated
  • Or because new technology in the hospital makes disposable items essential
  • Or because a referral hospital wrote a new prescription that was not anticipated when the indent was prepared
  • THE OTHER NOTES
    • Finance concurrence is not required for 7(a); for the others it is required above Rs 10,000 in each case
    • PRIOR APPROVAL of the medical officer in charge of the health unit or sub-divisional hospital is obtained in each case
    • In a divisional hospital it is the medical officer in charge of stores who approves
    • The emergency purchase power of every designated SAG officer of the medical department for medicine is RS 50,000 per case
    • A PAC, wherever applicable, is certified by the sanctioning authority before the purchase is made
    • Local purchase under 7(a) and (b) should not exceed 15% of the total annual budget in normal circumstances
    • Anything beyond 15% is kept to the minimum necessary, and has the PCMD's specific approval on recorded grounds
    So you should hold the 15% of the annual budget, and the Rs 50,000 emergency power.
मद 7 के नोट बताते हैं कि स्थानीय खरीद कब की जा सकती है।
  • वार्षिक मांगपत्रों के विरुद्ध आपूर्ति न होना या देर से होना
  • वार्षिक आपूर्ति निर्धारित समय से पहले खप जाना
  • किसी रोगी के लिए ब्रांड निष्ठा से किसी विशेष ब्रांड की आवश्यकता, भले दूसरा ब्रांड उपलब्ध हो
  • इसमें उपचार करने वाले चिकित्सक का अनुमोदन चाहिए, और यह प्रथा पूरी तरह टाली जाए, अन्यथा कम से कम रखी जाए
  • कम प्रयोग की, कम शेल्फ आयु वाली दवाएँ, जो इसीलिए भंडार में नहीं रखी जातीं
  • वे मदें जो मांगपत्र में इसलिए नहीं आईं कि वे नई तकनीक की मदें हैं
  • या इसलिए कि कोई नया रोग स्वरूप सामने आया जिसका अनुमान नहीं था
  • या इसलिए कि अस्पताल में आई नई तकनीक से डिस्पोजेबल मदें आवश्यक हो गईं

  • अन्य नोट
    • 7(a) में वित्त सहमति आवश्यक नहीं; शेष में हर मामले में 10,000 रुपये से ऊपर आवश्यक है
    • हर मामले में स्वास्थ्य इकाई या उप-मंडल अस्पताल के प्रभारी चिकित्सा अधिकारी का पूर्व अनुमोदन लिया जाता है
    • मंडल अस्पताल में यह अनुमोदन भंडार के प्रभारी चिकित्सा अधिकारी देते हैं
    • दवा के लिए चिकित्सा विभाग के हर नामित एसएजी अधिकारी की आपात खरीद शक्ति प्रति मामला 50,000 रुपये है
    • जहाँ लागू हो, पीएसी खरीद से पहले स्वीकृति प्राधिकारी प्रमाणित करते हैं
    • सामान्य परिस्थितियों में 7(a) और (b) की स्थानीय खरीद कुल वार्षिक बजट के 15% से अधिक नहीं होनी चाहिए
    • 15% से आगे जो भी हो वह न्यूनतम आवश्यक रहे, और उस पर दर्ज कारणों के साथ पीसीएमडी का विशेष अनुमोदन हो
    अर्थात् वार्षिक बजट का 15%, और 50,000 रुपये की आपात शक्ति आप रखिए।

Item 7(d) — EMERGENCY PURCHASE OF MEDICAL STORESमद 7(d) — चिकित्सा भंडार की आपात खरीद
Part (d) is the emergency purchase of items of medical stores and the repair of equipment. Those items are instruments, appliances, surgical dressings, X-ray films and the like.
  • PCMD: up to Rs 5 lakh per case
  • MD (Zonal Hospital) or SAG: up to Rs 50,000 per case; SG or JA Grade at headquarters: up to Rs 20,000 per case
  • CMS or CMO: up to Rs 50,000 per case
  • SG or JA Grade in a division: up to Rs 15,000 per item; Senior Scale: NIL
  • THE NOTES
    • PRIOR finance concurrence is not necessary
    • NO QUOTATIONS are required up to Rs 20,000 per item
    • Past purchase records are consulted to verify that the rate is reasonable
    • For such a purchase without quotation the prior approval of SAG officers is taken
    • Above Rs 20,000 the normal procurement process applies
    • The authority is PARA 14 and PARA 14.1 of the IRP 2002, with the Board's letter of 19.06.2008
    So Rs 20,000 is where quotations begin, and you should pair it with the past purchase record.
भाग (d) चिकित्सा भंडार की मदों की आपात खरीद और उपकरणों की मरम्मत है। वे मदें हैं उपकरण, साज-सामान, शल्य पट्टियाँ, एक्स-रे फिल्में और वैसी ही चीजें।
  • पीसीएमडी: प्रति मामला 5 लाख रुपये तक
  • एमडी (जोनल अस्पताल) या एसएजी: प्रति मामला 50,000 रुपये तक; मुख्यालय के एसजी या जेए ग्रेड: प्रति मामला 20,000 रुपये तक
  • सीएमएस या सीएमओ: प्रति मामला 50,000 रुपये तक
  • मंडल के एसजी या जेए ग्रेड: प्रति मद 15,000 रुपये तक; वरिष्ठ स्केल: शून्य
  • नोट
    • पूर्व वित्त सहमति आवश्यक नहीं है
    • प्रति मद 20,000 रुपये तक कोटेशन की आवश्यकता नहीं
    • दर उचित है या नहीं, यह जाँचने के लिए पिछली खरीद के अभिलेख देखे जाते हैं
    • बिना कोटेशन ऐसी खरीद पर एसएजी अधिकारियों का पूर्व अनुमोदन लिया जाता है
    • 20,000 रुपये से ऊपर सामान्य खरीद प्रक्रिया लागू होती है
    • प्राधिकार आईआरपी 2002 का पैरा 14 और पैरा 14.1 है, बोर्ड के 19.06.2008 के पत्र के साथ
    अर्थात् कोटेशन 20,000 रुपये से शुरू होते हैं, और उसके साथ पिछली खरीद का अभिलेख आप रखिए।
