Items 46 and 47 — JUNIOR RESIDENTS AND IMPLANTSमद 46 और 47 — कनिष्ठ रेजिडेंट और प्रत्यारोपण

Item 46 appoints Junior Residents, that is house surgeons.

• PCMD: FULL POWER; the DRM's column: NIL; SG or JA Grade: NIL; Senior Scale: NIL
• Finance concurrence IS necessary
• Extension of tenure beyond 2 YEARS is done only after the PCMD's approval
• The other conditions laid down in PARA 243 of the Medical Manual are followed

• ITEM 47 — MEDICAL IMPLANTS
  • It sanctions a works contract and purchase order for procurement of medical implants
  • Those are a hearing aid, a Bi-Pap or C-Pap and other implants, bought for supply or implant to patients, and other hospital related items
  • MD (Zonal Hospital): up to RS 5 LAKH for EACH ITEM, through quotation or tender
  • That is subject to an ANNUAL CEILING of RS 60 LAKH
  • CMS or CMO: up to Rs 5 lakh for each item through quotation or tender, on the same annual ceiling
  • SG or JA Grade: NIL; Senior Scale: NIL
  • Finance concurrence is taken wherever necessary
  • The cost ceiling is governed by the CGHS policies issued from time to time
  So an implant carries a per item figure and a yearly one, and you should quote both.

मद 46 कनिष्ठ रेजिडेंट, अर्थात् हाउस सर्जन, नियुक्त करती है।

• पीसीएमडी: पूर्ण शक्ति; मंडल रेल प्रबंधक का स्तंभ: शून्य; एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य
• वित्त सहमति आवश्यक है
• 2 वर्ष से आगे कार्यकाल का विस्तार पीसीएमडी के अनुमोदन के बाद ही होता है
• चिकित्सा नियमावली के पैरा 243 की अन्य शर्तें मानी जाती हैं

• मद 47 — चिकित्सा प्रत्यारोपण
  • यह चिकित्सा प्रत्यारोपण की खरीद के लिए कार्य संविदा और क्रय आदेश स्वीकृत करती है
  • वे हैं श्रवण यंत्र, बाई-पैप या सी-पैप और अन्य प्रत्यारोपण, जो रोगियों को देने या लगाने के लिए खरीदे जाते हैं, तथा अस्पताल से जुड़ी अन्य मदें
  • एमडी (जोनल अस्पताल): कोटेशन या निविदा से हर मद के लिए 5 लाख रुपये तक
  • यह 60 लाख रुपये की वार्षिक सीमा के अधीन है
  • सीएमएस या सीएमओ: कोटेशन या निविदा से हर मद के लिए 5 लाख रुपये तक, उसी वार्षिक सीमा पर
  • एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य
  • जहाँ आवश्यक हो वहाँ वित्त सहमति ली जाती है
  • लागत की सीमा समय-समय पर जारी सीजीएचएस नीतियों से चलती है
  अर्थात् प्रत्यारोपण में प्रति मद और प्रति वर्ष, दोनों आँकड़े हैं, और दोनों आप बताइए।

Item 48 — RE-ENGAGEMENT OF PART TIME DOCTORSमद 48 — अंशकालिक चिकित्सकों की पुनर्नियुक्ति

The item re-engages part-time dental surgeons, and part-time lady doctors, for a period UP TO 5 YEARS. The terms must not exceed ONE YEAR EACH.

• MD (Zonal Hospital): FULL POWERS
• DRM or CWM: FULL POWERS
• SG or JA Grade: NIL; Senior Scale: NIL

• THE NOTES
  • Finance concurrence IS necessary
  • The re-engagement is done on the recommendation of a nominated SAG medical officer of the zonal hospital, or the CMS or CMO
  • For a production unit the extension is with the approval of the GM or the CAO of that PU
  • Part-time lady doctors are re-engaged on lines similar to part-time dental surgeons
  • That is for hospitals and health units which do NOT have lady doctors
  So five years is reached one year at a time, and you should say the terms are yearly.

यह मद अंशकालिक दंत शल्य चिकित्सकों, और अंशकालिक महिला चिकित्सकों, को 5 वर्ष तक की अवधि के लिए फिर लगाती है। हर बार की अवधि एक वर्ष से अधिक नहीं होनी चाहिए।

  • पुनर्नियुक्ति जोनल अस्पताल के नामित एसएजी चिकित्सा अधिकारी, या सीएमएस अथवा सीएमओ की सिफारिश पर होती है
  • उत्पादन इकाई में विस्तार उस पीयू के महाप्रबंधक या सीएओ के अनुमोदन से होता है
  • अंशकालिक महिला चिकित्सक अंशकालिक दंत शल्य चिकित्सकों की तरह ही फिर लगाई जाती हैं
  • यह उन अस्पतालों और स्वास्थ्य इकाइयों के लिए है जहाँ महिला चिकित्सक नहीं हैं
  अर्थात् पाँच वर्ष एक-एक वर्ष करके पूरे होते हैं, और अवधि वार्षिक है — यह आप कहिए।

Item 49 — AMC FOR MEDICAL EQUIPMENTमद 49 — चिकित्सा उपकरणों की एएमसी

Part (a) enters into an AMC for medical equipment on SINGLE TENDER basis, placed on OEMs or authorised dealers.

• PCMD: FULL POWER; MD (Zonal Hospital): FULL POWER
• DRM, CWM, CMS or CMO: FULL POWER
• ADRM: up to Rs 2 lakh per case, with an annual ceiling of Rs 30 lakh
• SG or JAG: up to Rs 2 lakh per case, with an annual ceiling of Rs 30 lakh; Senior Scale: NIL

• PART (b) — AMC THROUGH LIMITED TENDER
  • PCMD: Rs 2 lakh each case per item, subject to a maximum ceiling of RS 50 LAKH PER ANNUM
  • MD (Zonal Hospital): up to Rs 1 lakh each case per item, ceiling Rs 20 lakh per annum
  • DRM, CWM or ADRM: up to Rs 1 lakh each case per item, ceiling Rs 20 lakh per annum
  • CMS or CMO: up to Rs 50,000 each case per item, ceiling Rs 10 lakh per annum
  • ACMS in charge of a sub-division: up to Rs 25,000 each case per item, ceiling Rs 5 lakh per annum
  • SG or JA Grade: NIL; Senior Scale: NIL
  So the single tender route is fuller than the limited one here, and you should notice that reversal.

भाग (a) चिकित्सा उपकरणों की एएमसी एकल निविदा के आधार पर करता है, जो ओईएम या अधिकृत विक्रेताओं को दी जाती है।

• पीसीएमडी: पूर्ण शक्ति; एमडी (जोनल अस्पताल): पूर्ण शक्ति
• मंडल रेल प्रबंधक, सीडब्ल्यूएम, सीएमएस या सीएमओ: पूर्ण शक्ति

• भाग (b) — सीमित निविदा से एएमसी
  • पीसीएमडी: प्रति मामला प्रति मद 2 लाख रुपये, प्रति वर्ष 50 लाख रुपये की अधिकतम सीमा के अधीन
  • एमडी (जोनल अस्पताल): प्रति मामला प्रति मद 1 लाख रुपये तक, प्रति वर्ष सीमा 20 लाख रुपये
  • मंडल रेल प्रबंधक, सीडब्ल्यूएम या एडीआरएम: प्रति मामला प्रति मद 1 लाख रुपये तक, प्रति वर्ष सीमा 20 लाख रुपये
  • सीएमएस या सीएमओ: प्रति मामला प्रति मद 50,000 रुपये तक, प्रति वर्ष सीमा 10 लाख रुपये
  • उप-मंडल के प्रभारी एसीएमएस: प्रति मामला प्रति मद 25,000 रुपये तक, प्रति वर्ष सीमा 5 लाख रुपये
  • एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य
  अर्थात् यहाँ एकल निविदा का रास्ता सीमित निविदा से भरा हुआ है, और यह उलटफेर आप देखिए।

Item 49 — THE NOTES ON A MEDICAL AMCमद 49 — चिकित्सा एएमसी के नोट

The eight notes under item 49 decide how a medical AMC is awarded and watched.

• An AMC on single tender basis may be awarded on PROPRIETARY ARTICLE CERTIFICATE items
• Since AMCs are in the nature of service contracts, and OEMs are often reluctant, earnest money or security deposit need not be insisted upon
• Care is taken to contract only with reputed dealers, for satisfactory performance and minimum downtime
• The powers are exercised subject to availability of funds, and with the PRIOR concurrence of finance
• As far as possible the AMC or CMC is done through the OEM or its authorised service providers
• Where that is not possible the reasons are recorded IN WRITING, and it may be got done from reputed dealers
• As far as possible all similar items are CLUBBED under one AMC
• For a single tender the tender committee and the accepting authority are ONE STEP HIGHER than for an open tender
• That step up does not apply where the GM is the accepting authority
• The accepting authority undertakes a MONTHLY REVIEW of the works authorised
• Each officer maintains a REGISTER showing full particulars of the works he authorised
• The register is open to verification by accounts while passing the bills, and the annual ceilings are watched through it
• While seeking concurrence the register is provided with the file

So the register carries the ceiling, and you should send it with the file to finance.मद 49 के आठ नोट तय करते हैं कि चिकित्सा एएमसी कैसे दी जाए और उस पर नजर कैसे रखी जाए।

• एकल निविदा की एएमसी स्वामित्व वस्तु प्रमाणपत्र वाली मदों पर दी जा सकती है
• एएमसी सेवा संविदा की प्रकृति की है, और ओईएम अक्सर हिचकते हैं, इसलिए बयाना या प्रतिभूति जमा पर जोर देना आवश्यक नहीं
• संतोषजनक निष्पादन और कम ठहराव के लिए संविदा केवल प्रतिष्ठित विक्रेताओं से की जाती है
• ये शक्तियाँ निधि की उपलब्धता के अधीन, और वित्त की पूर्व सहमति से चलती हैं
• यथासंभव एएमसी या सीएमसी ओईएम या उसके अधिकृत सेवा प्रदाताओं से कराई जाती है
• जहाँ यह संभव न हो, वहाँ कारण लिखित में दर्ज होते हैं, और यह प्रतिष्ठित विक्रेताओं से कराई जा सकती है
• यथासंभव सभी समान मदें एक ही एएमसी में मिला दी जाती हैं
• एकल निविदा में निविदा समिति और स्वीकृति प्राधिकारी खुली निविदा से एक पायदान ऊपर होते हैं
• जहाँ महाप्रबंधक स्वीकृति प्राधिकारी हों, वहाँ यह पायदान नहीं लगता
• स्वीकृति प्राधिकारी अधिकृत कार्यों की मासिक समीक्षा करता है
• हर अधिकारी अपने अधिकृत कार्यों का पूरा विवरण दिखाने वाला रजिस्टर रखता है
• बिल पारित करते समय लेखा उस रजिस्टर की जाँच कर सकता है, और वार्षिक सीमाएँ उसी से देखी जाती हैं
• सहमति माँगते समय रजिस्टर फाइल के साथ दिया जाता है

अर्थात् सीमा रजिस्टर में रहती है, और उसे फाइल के साथ वित्त को आप भेजिए।
