Item 17 holds medical boards for decategorisation and invalidation.

• (i) For AEMG: PCMD FULL POWERS; MD (Zonal Hospital) FULL POWERS
• CMS or CMO: FULL POWERS; JAG or SG divisional in-charges ONLY: FULL POWERS; Senior Scale: NIL
• (ii) For INVALIDATION: PCMD FULL POWERS; the DRM's column NIL; SG or JA Grade NIL; Senior Scale NIL
• Finance concurrence is NOT necessary
• The exercise of these powers by the MD, CMS or CMO is subject to STANDING MEDICAL BOARDS being nominated by the PCMD, if none exists
• The authority is PARA 622 of the IRMM

• ITEM 18 — NAKED EYE VISION
  • Relaxation of the medical board in the matter of naked eye vision
  • PCMD: AS PER PARA 512 of the IRMM; the DRM's column: NIL; the other columns: NIL
  • The authority is PARA 512 NOTE 8 of the IRMM
  • The Board's letters of 06.04.99 and 31.12.96 are printed in the same block of the schedule
  So invalidation is the PCMD's alone, and you should not read a CMS into it.

• एमडी, सीएमएस या सीएमओ की ये शक्तियाँ इस शर्त पर हैं कि स्थायी चिकित्सा बोर्ड न हों तो पीसीएमडी उन्हें नामित करें
• प्राधिकार आईआरएमएम का पैरा 622 है

• मद 18 — नग्न आँख दृष्टि
  • नग्न आँख दृष्टि के मामले में चिकित्सा बोर्ड की छूट
  • पीसीएमडी: आईआरएमएम के पैरा 512 के अनुसार; मंडल रेल प्रबंधक का स्तंभ: शून्य; शेष स्तंभ: शून्य
  • प्राधिकार आईआरएमएम का पैरा 512 नोट 8 है
  • बोर्ड के 06.04.99 और 31.12.96 के पत्र अनुसूची के उसी खंड में छपे हैं
  अर्थात् अमान्यकरण केवल पीसीएमडी का है, और उसमें सीएमएस आप न पढ़िए।

Item 19 — ACCEPTANCE OF GIFTS BY A RAILWAY HOSPITALमद 19 — रेल अस्पताल द्वारा उपहार स्वीकार करना

The item accepts gifts by railway hospitals on behalf of the medical department.

• PCMD: FULL POWERS; MD (Zonal Hospital): FULL POWERS
• SAG, SG, JAG, Senior Scale and Junior Scale at headquarters: FULL POWERS
• CMS or CMO: FULL POWERS

• THE NOTES
  • Finance concurrence is NOT necessary
  • The PCMD, the PFA and the GM are ADVISED of such gifts
  • The gift is advised as part of the hospital's DEAD STOCK
  So a gift is taken freely but never quietly, and you should name the three who are told.

यह मद चिकित्सा विभाग की ओर से रेल अस्पतालों द्वारा उपहार स्वीकार करने की है।

• पीसीएमडी: पूर्ण शक्तियाँ; एमडी (जोनल अस्पताल): पूर्ण शक्तियाँ
• मुख्यालय के एसएजी, एसजी, जेएजी, वरिष्ठ स्केल और कनिष्ठ स्केल: पूर्ण शक्तियाँ

• नोट
  • वित्त सहमति आवश्यक नहीं है
  • ऐसे उपहारों की सूचना पीसीएमडी, पीएफए और महाप्रबंधक को दी जाती है
  • उपहार अस्पताल के डेड स्टॉक के भाग के रूप में बताया जाता है
  अर्थात् उपहार खुलकर लिया जाता है पर चुपचाप नहीं, और तीनों के नाम आप बताइए।

Item 20(a) — SPECIAL DIAGNOSTIC INVESTIGATIONSमद 20(a) — विशेष निदान जाँचें

The item sanctions special diagnostic investigations, a CT scan and an MRI included, in non-railway identified institutions. It is for service or consultation rendered in cases the authorised medical attendant has referred.

• PCMD, or the CMO of a production unit: FULL POWERS
• MD (Zonal Hospital): FULL POWERS up to RS 15,000 in each test
• CMS: up to RS 15,000 in each test
• SG or JA Grade: up to RS 10,000 in each test; Senior Scale: NIL

• THE NOTES
  • Finance concurrence is necessary for IDENTIFYING and FIXING RATES with such institutions
  • A SIX-MONTHLY statement of such expenses is sent to the PCMD
  • The power does NOT apply to artificial aids like hearing aids, prosthetic valves for the heart or sophisticated artificial limbs
  • Beyond Rs 15,000, associate finance's concurrence and the approval of the PCMD or the CMO of the zonal railway or PU is required
  • The rates are governed by the notified CGHS rates
  • A health unit with ONE doctor carries an imprest of RS 30,000, subject to a maximum of Rs 1,500 per investigation
  • A health unit with MORE THAN ONE doctor carries an imprest of RS 60,000, on the same per investigation limit
  • The TREATING DOCTOR recommends the investigation
  • The MD, CMO, CMS, or another doctor they nominate, accords the approval
  So Rs 15,000 is where finance and the PCMD enter, and you should carry the CGHS rate with it.

यह मद गैर-रेल चिह्नित संस्थानों में विशेष निदान जाँचें स्वीकृत करती है, सीटी स्कैन और एमआरआई भी। यह उन मामलों की सेवा या परामर्श के लिए है जिन्हें अधिकृत चिकित्सा परिचारक ने भेजा हो।

• पीसीएमडी, या उत्पादन इकाई के सीएमओ: पूर्ण शक्तियाँ
• एमडी (जोनल अस्पताल): हर जाँच में 15,000 रुपये तक पूर्ण शक्तियाँ
• सीएमएस: हर जाँच में 15,000 रुपये तक
• एसजी या जेए ग्रेड: हर जाँच में 10,000 रुपये तक; वरिष्ठ स्केल: शून्य

• नोट
  • ऐसे संस्थानों को चिह्नित करने और उनकी दरें तय करने में वित्त सहमति आवश्यक है
  • ऐसे व्यय का छमाही विवरण पीसीएमडी को भेजा जाता है
  • यह शक्ति कृत्रिम सहायक उपकरणों पर नहीं चलती, जैसे श्रवण यंत्र, हृदय के कृत्रिम वाल्व या उन्नत कृत्रिम अंग
  • 15,000 रुपये से आगे सहयोगी वित्त की सहमति और जोनल रेलवे या पीयू के पीसीएमडी अथवा सीएमओ का अनुमोदन चाहिए
  • दरें अधिसूचित सीजीएचएस दरों से चलती हैं
  • एक चिकित्सक वाली स्वास्थ्य इकाई का अग्रदाय 30,000 रुपये है, प्रति जाँच अधिकतम 1,500 रुपये के अधीन
  • एक से अधिक चिकित्सक वाली स्वास्थ्य इकाई का अग्रदाय 60,000 रुपये है, प्रति जाँच उसी सीमा पर
  • जाँच की सिफारिश उपचार करने वाले चिकित्सक करते हैं
  • अनुमोदन एमडी, सीएमओ, सीएमएस, या उनके नामित किसी अन्य चिकित्सक से मिलता है
  अर्थात् 15,000 रुपये पर वित्त और पीसीएमडी आते हैं, और उसके साथ सीजीएचएस दर आप रखिए।

Item 20(b) — REFERRAL FOR A PET SCANमद 20(b) — पेट स्कैन के लिए रेफरल

The item refers railway beneficiaries to non-railway medical institutions for a PET SCAN.

• PCMD, or the CMO of a production unit: FULL POWERS
• MD (Zonal Hospital): FULL POWERS, as detailed in the remarks column
• CMS or equivalent: FULL POWERS, as detailed in the remarks column
• CMS or Sr DMO: FULL POWERS, as detailed in the remarks column; Senior Scale: NIL

• THE LIMIT OF SANCTION
  • It is limited to the NABL or non-NABL rate that CGHS specifies for that city
  • Where the scan is done elsewhere, the rate for the city nearby the one where it is to be or has been done applies
  • In exceptional cases, recorded IN WRITING, the PET scan rates may exceed the prescribed CGHS rates
  • That is where no hospital agrees to conduct the PET scan on CGHS rates
  • THREE QUOTATIONS are then called for
  • A MEDICAL BOARD allows the scan at the institution quoting the LOWEST rate
  • That carries the concurrence of associate finance and the approval of the CMD of the zone or the CMO of the PU
  • The treating doctor recommends the scan
  So a PET scan above the CGHS rate needs three quotations, and you should name the board that allows it.

यह मद रेल लाभार्थियों को पेट स्कैन के लिए गैर-रेल चिकित्सा संस्थानों में भेजती है।

• पीसीएमडी, या उत्पादन इकाई के सीएमओ: पूर्ण शक्तियाँ

• स्वीकृति की सीमा
  • यह उस शहर के लिए सीजीएचएस की निर्दिष्ट एनएबीएल या गैर-एनएबीएल दर तक सीमित है
  • जहाँ स्कैन अन्यत्र हो, वहाँ उस शहर के पास के शहर की दर लगती है जहाँ वह होना है या हुआ है
  • असाधारण मामलों में, लिखित में दर्ज करके, पेट स्कैन की दरें निर्धारित सीजीएचएस दरों से अधिक हो सकती हैं
  • यह वहाँ है जहाँ कोई अस्पताल सीजीएचएस दरों पर पेट स्कैन करने को तैयार न हो
  • तब तीन कोटेशन बुलाए जाते हैं
  • चिकित्सा बोर्ड सबसे कम दर देने वाले संस्थान में स्कैन की अनुमति देता है
  • इसके साथ सहयोगी वित्त की सहमति और जोन के सीएमडी या पीयू के सीएमओ का अनुमोदन होता है
  • स्कैन की सिफारिश उपचार करने वाले चिकित्सक करते हैं
  अर्थात् सीजीएचएस दर से ऊपर पेट स्कैन में तीन कोटेशन चाहिए, और अनुमति देने वाला बोर्ड आप बताइए।
