Item 5(a) — REIMBURSEMENT UNDER NORMAL CIRCUMSTANCESमद 5(a) — सामान्य परिस्थितियों में प्रतिपूर्ति

Item 5 sanctions reimbursement of medical expenses for treatment, a pacemaker included, of railway servants, RELHS-97 beneficiaries and members of their families. Part (a) is the normal case, where an AUTHORISED railway medical officer refers the patient. The reference is to a Government hospital, autonomous body hospitals included, and the treatment proceeds by his advice.

• PCMD and SAG: FULL POWERS; MD (Zonal Hospital): FULL POWERS
• DRM, ADRM, CWM or SAG: FULL POWERS
• SG or JA Grade: up to RS 30,000
• Senior Scale and Assistant Scale officers: NIL

• THE NOTES
  • Finance concurrence is NOT necessary in part (a)
  • The limits of rates for a pacemaker, stents and the like are the CGHS RATES issued from time to time
  • For the GM's and AGM's powers the Board's letter of 18.10.2017 is referred to
  • The powers shown are for officers of the MEDICAL DEPARTMENT only
  • DRM, ADRM or CWM exercise the column 4 powers where NO SAG officer of the medical department is available
  So the normal case turns on the reference, and you should look for the authorised medical officer.

मद 5 रेल सेवकों, आरईएलएचएस-97 लाभार्थियों और उनके परिवार के सदस्यों के उपचार, पेसमेकर सहित, के चिकित्सा व्यय की प्रतिपूर्ति स्वीकृत करती है। भाग (a) सामान्य स्थिति है, जहाँ अधिकृत रेल चिकित्सा अधिकारी रोगी को रेफर करते हैं। रेफरेंस सरकारी अस्पताल को होता है, स्वायत्त निकाय के अस्पताल भी, और उपचार उनकी सलाह से चलता है।

• वरिष्ठ स्केल और सहायक स्केल अधिकारी: शून्य

• नोट
  • भाग (a) में वित्त सहमति आवश्यक नहीं है
  • पेसमेकर, स्टेंट आदि की दरों की सीमा समय-समय पर जारी सीजीएचएस दरें हैं
  • महाप्रबंधक और अपर महाप्रबंधक की शक्तियों के लिए बोर्ड का 18.10.2017 का पत्र देखा जाता है
  • दिखाई गई शक्तियाँ केवल चिकित्सा विभाग के अधिकारियों के लिए हैं
  • जहाँ चिकित्सा विभाग का कोई एसएजी अधिकारी उपलब्ध न हो, वहाँ स्तंभ 4 की शक्तियाँ मंडल रेल प्रबंधक, एडीआरएम या सीडब्ल्यूएम प्रयोग करते हैं
  अर्थात् सामान्य स्थिति रेफरेंस पर टिकी है, और अधिकृत चिकित्सा अधिकारी आप खोजिए।

Item 5(b) — TREATMENT TAKEN IN EMERGENCYमद 5(b) — आपात में लिया गया उपचार

Part (b) covers treatment taken in an emergency WITHOUT a proper reference by an authorised railway medical officer.

• (i) PRIVATE NON-RECOGNISED OR RECOGNISED HOSPITALS
  • PCMD: Rs 5 lakh in each case; MD (Zonal Hospital): Rs 2 lakh per case without annual ceiling
  • DRM or CWM: Rs 2 lakh per case; CMS or CMO: Rs 1 lakh per case without annual ceiling
  • SG or JA Grade: NIL; Senior Scale: NIL
  • The AGM has RS 10 LAKH in each case for (b)(i), and the GM has FULL POWERS for both (b)(i) and (b)(ii)
• (ii) A GOVERNMENT HOSPITAL, AUTONOMOUS BODY HOSPITALS INCLUDED
  • Those are hospitals fully funded from Government funds but given autonomy for administrative purposes, such as AIIMS or NIMS Hyderabad
  • PCMD: FULL POWERS; MD (Zonal Hospital): Rs 2 lakh per case without annual ceiling
  • DRM or CWM: Rs 5 lakh per case; CMS or CMO: Rs 1 lakh per case without annual ceiling
  • SG or JA Grade: NIL; Senior Scale: NIL; the AGM has FULL POWERS for (b)(ii)
  • Concurrence of associate finance IS required in part (b)
  So without a reference the Government hospital carries the larger power, and you should notice which way it runs.

  • अपर महाप्रबंधक को (b)(i) में हर मामले पर 10 लाख रुपये, और महाप्रबंधक को (b)(i) तथा (b)(ii) दोनों में पूर्ण शक्तियाँ हैं
• (ii) सरकारी अस्पताल, स्वायत्त निकाय के अस्पताल भी
  • वे ऐसे अस्पताल हैं जो पूरी तरह सरकारी निधि से चलते हैं पर प्रशासनिक प्रयोजन से स्वायत्त हैं, जैसे एम्स या निम्स हैदराबाद
  • पीसीएमडी: पूर्ण शक्तियाँ; एमडी (जोनल अस्पताल): बिना वार्षिक सीमा प्रति मामला 2 लाख रुपये
  • मंडल रेल प्रबंधक या सीडब्ल्यूएम: प्रति मामला 5 लाख रुपये; सीएमएस या सीएमओ: बिना वार्षिक सीमा प्रति मामला 1 लाख रुपये
  • एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य; अपर महाप्रबंधक को (b)(ii) में पूर्ण शक्तियाँ हैं
  • भाग (b) में सहयोगी वित्त की सहमति आवश्यक है
  अर्थात् रेफरेंस के बिना सरकारी अस्पताल में शक्ति बड़ी है, और वह किस ओर चलती है — यह आप देखिए।

Item 5(c) and (d) — ADVANCE PAYMENT ON A REFERRALमद 5(c) और (d) — रेफरल पर अग्रिम भुगतान

Part (c) is advance payment on estimate to Government and other autonomous body hospitals, to sanction a referral in an emergency of railway beneficiaries.

• AGM: FULL POWERS; PCMD: FULL POWERS
• MD (Zonal Hospital): Rs 1 lakh per case without annual ceiling
• DRM or CWM: up to Rs 5 lakh in each case; CMS or CMO: Rs 1 lakh per case without annual ceiling
• SG or JA Grade: NIL; Senior Scale: NIL

• PART (d) — ADVANCE TO A PRIVATE NON-RECOGNISED HOSPITAL
  • It is for treatment on a referral in an emergency, where the treatment is available NEITHER at a railway hospital NOR at a recognised hospital
  • AGM: up to Rs 10 lakh in each case; PCMD: up to Rs 5 lakh in each case
  • MD (Zonal Hospital): up to Rs 1 lakh in each case without annual ceiling
  • DRM or CWM: up to Rs 2 lakh in each case; CMS or CMO: up to Rs 1 lakh in each case without annual ceiling
  • The GM has FULL POWERS; SG or JA Grade: NIL; Senior Scale: NIL
  • Finance concurrence IS necessary in both parts
  So the private hospital's advance rests on a double absence, and you should state both.

भाग (c) सरकारी और अन्य स्वायत्त निकाय के अस्पतालों को अनुमान पर अग्रिम भुगतान है, जिससे आपात में रेल लाभार्थियों का रेफरल स्वीकृत हो।

• अपर महाप्रबंधक: पूर्ण शक्तियाँ; पीसीएमडी: पूर्ण शक्तियाँ

• भाग (d) — निजी अमान्य अस्पताल को अग्रिम
  • यह आपात के रेफरल पर उपचार के लिए है, जहाँ वह उपचार न रेल अस्पताल में हो और न किसी मान्य अस्पताल में
  • अपर महाप्रबंधक: हर मामले में 10 लाख रुपये तक; पीसीएमडी: हर मामले में 5 लाख रुपये तक
  • एमडी (जोनल अस्पताल): बिना वार्षिक सीमा हर मामले में 1 लाख रुपये तक
  • मंडल रेल प्रबंधक या सीडब्ल्यूएम: हर मामले में 2 लाख रुपये तक; सीएमएस या सीएमओ: बिना वार्षिक सीमा हर मामले में 1 लाख रुपये तक
  • महाप्रबंधक को पूर्ण शक्तियाँ हैं; एसजी या जेए ग्रेड: शून्य; वरिष्ठ स्केल: शून्य
  • दोनों भागों में वित्त सहमति आवश्यक है
  अर्थात् निजी अस्पताल का अग्रिम दो अनुपस्थितियों पर टिका है, और दोनों आप बताइए।

Item 5(e) — ADVANCE FOR A KIDNEY TRANSPLANTमद 5(e) — गुर्दा प्रत्यारोपण का अग्रिम

Part (e) sanctions advance payment for the TRANSPLANT OF A KIDNEY.

• GM: FULL POWERS, subject to CGHS policy
• The other columns of the schedule carry no entry for this part
• Finance concurrence IS necessary
• The authority is the Board's letter of 18.10.2017

So a kidney transplant advance stops at the GM, and you should tie it to CGHS policy.भाग (e) गुर्दा प्रत्यारोपण के लिए अग्रिम भुगतान स्वीकृत करता है।

• महाप्रबंधक: सीजीएचएस नीति के अधीन पूर्ण शक्तियाँ
• इस भाग में अनुसूची के अन्य स्तंभों में कोई प्रविष्टि नहीं है

अर्थात् गुर्दा प्रत्यारोपण का अग्रिम महाप्रबंधक पर रुकता है, और उसे आप सीजीएचएस नीति से जोड़िए।
